For your patients to qualify for the free CaptionCall phone and service, they must obtain certification from a hearing-care or healthcare professional indicating they have hearing loss and need CaptionCall to communicate effectively by phone.

Patient Eligibility

 

Anyone who has trouble using the telephone due to hearing loss is a possible candidate for CaptionCall. There is no predetermined decibel threshold to qualify. Your patient must simply have a hearing loss that, in your professional opinion, necessitates the use of captioned telephone service to communicate effectively by phone.

  • To use the CaptionCall phone, a qualified individual will need a standard phone line and an internet connection.
  • To use CaptionCall Mobile, they will need a CaptionCall account, an Apple iPad 2 or later, and a Wi-Fi or cellular internet connection.

How to Order CaptionCall

 

You order the CaptionCall phone for your patients simply by submitting the Professional Certification Form on their behalf. There are two easy ways to submit the Certification Form:

Option 1:  Complete the CaptionCall Certification Form using the CaptionCall module for Noah 4. This allows you to submit the form with a few simple keystrokes. Watch the video to learn how. Fill out the form below and click the 'Download' button to download the CaptionCall NOAH 4 Module (.zip), or contact your local representative for assistance.

Option 2:  Complete a hard or soft copy of the Professional Certification Form and fax to 1-888-531-1906 or email to certification@captioncall.com. (These pre-populated forms are provided for you when you signup to share CaptionCall.)

Once we receive the signed Certification Form, a CaptionCall representative will contact your referred patient to schedule their delivery and installation of a complimentary CaptionCall phone and free captioning service.
